Emergency Contact Form
If something should happen to you, would the church know how to get in contact with your family members? There are times when the church is called upon to make connections in the event of a medical emergency. To assist in this, we have created an Emergency Contact Form that you may fill out so we might keep it on file here at CPUCC should the need arise for us to assist you. Fill out as much of the form as you feel comfortable doing, and we will keep it on file. By preparing for a future event now, you will make it easier for your spiritual home to care for your loved ones.

Personal Pre-Planning Your Funeral or Memorial Service
The truth is we all die AND very few of us like to talk about death. However, it can be very helpful to your church, your family and friends, if they know your wishes. To assist in this, we have created a Personal Pre-planning Funeral or Memorial Service Form (see below) that you may complete, and help to have conversations happen between you and your family and your church. Fill out as much of the form as you feel comfortable doing, and if you wish we will keep a copy on file. By preparing for a future event now, you will make it easier for your spiritual home to care for you and your loved ones.

Advance Health Care Directive
This form (3-pages) lets you give specific instructions about any aspect of your health care. Choices are provided for you to express your wishes regarding the provision, withholding, or withdrawal of treatment to keep you alive, as well as the provision of pain relief. Space is provided for you to add to the choices you have made or for you to write out any additional wishes. This form also lets you express an intention to donate your bodily organs and tissues following your death. Lastly, this form lets you designate a physician to have primary responsibility for your health care.
After completing this form, sign and date the form at the end. The form must be signed by two qualified witnesses or acknowledged before a notary public. Give a copy of the signed and completed form to your physician, to any other health care providers you may have, to any health care institution at which you are receiving care, and to any health-care agents you have named.
Sample Codicil Language for a Will
First, the definition: A codicil is a supplement that explains or modifies a will, or revokes part of one.
Codicils should be used for relatively minor changes to a will, such as:
- Adding or deleting a specific bequest
- Changing a Personal Representative, or
- Updating a legal name of someone listed in the will
IMPORTANT NOTE: Our church staff does not create the codicil. Our church is not in the business of providing legal or tax advisory services. We encourage you to talk with your legal counsel directly. State laws govern these kinds of documents, and your attorney will know state laws better than we do.
